Position Description: Nurse Practitioner

The Health Center Nurse Practitioner supports our Community Health Center mission to improve the wellbeing of the community we serve by providing accessible, high-quality healthcare. The Health Center Nurse Practitioner provides high-quality primary health care services, emphasizing the prevention, treatment of disease and management of chronic illness, and is responsible for the delivery of medical care within the legal parameters established by the Illinois Medical Practice Act.

ESSENTIAL DUTIES & RESPONSIBILITIES

• Serves as an essential member of the health center medical provider team, interfacing directly with patients and other health center team members to carry out the mission, vision and values of Thresholds Health, in compliance with the rules and regulations governing HRSA FQHC Look Alike community health centers.

• Facilitates entry of the client into the health care system by assessing the primary health care needs, including comprehensive health histories, performing physical examinations, ordering and/or performing pertinent diagnostic tests, analyzing collected data to determine client health status and formulating a problem list.

• Provides coordinated, continuous and culturally competent primary health care to individuals that span the health continuum through mutual goal setting and planning. Prescribes a variety of treatment modalities to meet health and illness needs. Collaborates with other staff to assure that health teaching and counseling is provided. Manages and evaluates the plan of care, process and outcome and then modifies the plan as needed. Provides both in-person and telehealth medical encounters.

• Documents comprehensive, accurate and continual data on clients’ records and program reports according to Organization standards in an electronic health record. Strictly adheres to privacy and confidentiality in all written and verbal communication regarding patient care both with internal and external team members and consultants.

• Provides high-quality care in an efficient manner and meets expected productivity levels. Minimizes waste of materials. Keeps work areas (patient treatment rooms and office spaces) organized, clean and free of hazards.

• Adheres to collaborative practice principles of respect and joint decision making when providing clinical supervision of the medical care delivered and consulting with team members about managing the client treatment plan. Follows clinical practice standards related to their role and responsibilities.

• Participates in bi-directional quality improvement activities through protocol development and collaborative peer review. Within peer review evaluates the care provided by colleagues based on clinical standards. Implements recommended changes and/or improvements to treatment approaches and/or documentation based on the evaluative process provided by peers.

• Practices within the legal and ethical guidelines established by the Medical Practice Act, the Physicians Code of Ethics and the Specialty Standards of Practice.

• Adheres to professional standards as outlined by governmental bodies, American Medical Association(AMA), American College of Obstetrics and Gynecology (ACOG), American College of Physicians, American Academy of Pediatrics, American Academy of Family Physicians, and guidelines of private funders and public agencies. Participates in periodic evaluative reviews and/or in-house and external staff training to ensure that he/she understands and continues to adhere to such standards.

• Initiates requests for assistance or guidance to address new issues or complex concepts affecting adherence to professional standards.

• Supervisory Responsibilities: This position requires supervision of ancillary staff within the care of an individual patient.

• Performs other duties as necessary.

• Reasoning Ability: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables. Education and/or Experience:

• Nurse Practitioner (NP) or Advanced Practice Nurse (APN)Certificates, Licenses, Registrations: Active Illinois licensure, documentation of a conferred degree Language Skills: Spanish and American Sign Language are helpful but not required.

• Ability to read, analyze, and interpret the most complex documents.

• Ability to respond effectively to the most sensitive inquiries or complaints. General

• Mathematical Skills: Ability to work with mathematical concepts such a probability and statistical inference, and fundamentals of plane and solid geometry and trigonometry. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

• Familiarity with community healthcare delivery Ability to use computer software/applications to receive, organize, prioritize and transmit information required
